Position Summary:
4 Your Peace of Mind, LLC is seeking a compassionate and reliable Licensed Practical Nurse (LPN) for a private duty Per Diem assignment in Bolton, CT. The LPN will provide direct, patient-centered care under RN supervision in accordance with the Connecticut Nurse Practice Act and agency policies.
4 Your Peace of Mind, LLC is seeking a compassionate and reliable Licensed Practical Nurse (LPN) for a private duty Per Diem assignment in Bolton, CT. The LPN will provide direct, patient-centered care under RN supervision in accordance with the Connecticut Nurse Practice Act and agency policies.
Qualifications:
- Active, unrestricted Connecticut LPN license
- Graduate of an accredited practical nursing program
- 1+ year of home health or related experience preferred
- Reliable vehicle, valid driver’s license, and auto insurance
- Strong communication, documentation, and time management skills
Responsibilities:
- Provide hands-on patient care and education
- Administer medications and treatments per orders
- Document all visits and MARs accurately within 24 hours
- Report any changes in patient condition to supervising RN
- Adhere to infection control and safety protocols
- Use EVV system for visit verification
Location: Bolton, CT
Schedule: Monday–Friday, 8:30 AM–4:30 PM

Compensation: $40.00 per hour
